Breastfeeding case study of Baby Aarya – 5 weeks, from Hemisha

Birth

  • Delivered at 40 weeks 1 day – due date 19th Dec arrived 20th Dec 7:30am.
  • Induced on 19th Dec 1:40am, went to hospital with pain – on swab test confirmed waters had broken although I had no sign of this. Antibiotics drip given and induction started.
  • Upon arriving at hospital I was 0 percent dilated. Epidural given at 2 cm dilated.
  • Temp went up in labour – antibiotics given orally after delivery.
  • Discharged on 21st Dec, breast feeding specialist at the hospital did come to have a chat and help but baby latched for 2-3 min and came off shortly after. No further help was given.

Since birth

  • Suffered from haemorrhoids, with extreme pressure and pain and was triaged at hospital. Had a lot of pain after bowel movement and could not express or pump much and could not try to get baby to latch or provide much skin to skin.
  • Formula bottle introduced from day 3 of birth.
  • Shortly after this I contracted covid which made me feel even weaker and symptoms lasted 12 days. 1 vaccine was taken at time of contracting covid.
  • Staying with my parents currently receiving great help and support
  • I really wanted to breastfeed my little one but encountered various circumstances within the last few weeks. Feeling quite low and stressed that I am not able to get my baby to latch. She roots to the breast when she’s hungry but can accept the nipple and immediately comes away despite trying.

Current schedule

  • No real schedule at the moment- each day seems different.
  • Aarya normally wakes up at 7am- 8am.
  • Currently expressing with very little in 30min maybe 1 oz collected. If I express again in 2 hours not much comes out.
  • Baby is feeding on Aptamil powder formula. Much more formula than expressed breast milk as not producing much via expressing.
  • Usually 8 wet diapers a day and 4 to 5 dirty nappies per day.
  • Quite gassy and on passing bowel movement there is quite a lot of straining.
  • Colic sometimes due to drinking fast when hungry but generally does pace herself for the feed. Tommee tippee antic-colic bottles used.
  • Currently feeding on demand 3 oz seems to be the average amount per feed. Averaging 600ml per day.
  • Alert and playful for 3 to 3 hours per day.
  • Wakes up every 3 or 4 hours at night – formula given and goes back to sleep.
  • Baby sleeps on moses basket but needs a little rocking and then is settled in the moses basket.

Other notes

  • I would really like to breast feed my baby, however I have lost confidence as she is not able to latch despite efforts over the last 8 days from various sources such as different breast feeding positions and also health visitors.
  • Want to be able to breast feed baby and also get enough expressed milk to give via bottle.
  • Have not tried nipple shields yet.
  • Concerned I have low milk supply and how to improve this.
  • Currently pumping when can, around 4 times a day.
